Accommodation Requests

In accordance with federal rights laws, including Title I of the Americans with Disabilities Act of 1990 or ADA and Title VI of the Civil Rights Act of 1964, Lakewood Water District will not discriminate on the basis of race, color, sex, national origin, age, income, disability, or limited English proficiency in its services, programs, or activities.

The District will generally, upon request, provide appropriate aids, services, and modifications leading to effective communication for qualified individuals with disabilities or those who have trouble speaking or understanding English so they can participate equally in the District’s programs, services, and activities.

Accommodations include qualified sign or other language interpreters, written information in other languages or other formats like Braille, large print, and audio, and other ways of making information and communications accessible to people who have limited English proficiency, speech, hearing, or vision impairments.

Anyone who requires an auxiliary aid or service for effective communication, a modification of policies or procedures, or translation services to participate in a District program, service, or activity should contact the Civil Rights Compliance Coordinator as soon as possible but no later than 48 hours before the scheduled event.

The District’s Civil Rights Compliance Coordinator will respond to the request within 15 calendar days or in advance of a scheduled meeting or event. If the response does not satisfactorily resolve the issue, the requestor may file a formal complaint with the District. All requests for accommodation and alternate formats will be kept on file for at least three years.

Requests for accommodation at a District meeting or event should include:

  • The requestor’s name, address, email, and telephone number (if any).
  • A description of the program, service, or activity where accommodation is required.
  • The location of the program, service, or activity.
  • What type of accommodation is desired.
  • A brief description of why the accommodation is needed.

Requests for materials in alternate formats should include:

  • The requestor’s name, address, email, and telephone number (if any).
  • The name or description of the District document or materials to be reformatted.
  • Which type of format is desired (e.g., Braille, large format, audio recording, etc.)
  • A brief description of why the alternate format is needed.
